Directions: This form will be used by all staff who wish to request tuition reimbursement for graduate, undergraduate, or certification courses. The class must be approved prior to registration for the course.

In order to receive reimbursement, staff must submit the following to the superintendent's office:

  • proof of completion
  • proof of payment by the employee
  • copy of this form
