Activities Transportation Release

This form is needed if a student is to be released at an activity to anyone other than a parent/legal guardian OR will be traveling to and from an activity with a parent/legal guardian. All requests must be approved by the office in advance of the activity.

We highly recommend that all participants ride district-issued transportation to away contests as it provides efficiency, camaraderie, and timeliness for our extracurricular activities. Requests made to transport your child to an activity will require an exception above normal circumstances.

I understand that Hot Springs County School District #1 is relieved of all responsibility concerning my child when I release them to a driver other than his/her guardian/parent and that I must have approval from Hot Springs County High School or Thermopolis Middle School each time I wish to have my child released from riding the bus.
